The 2026 Ebola outbreak is the fastest-spreading on record. The U.S. dismantled the systems built to stop it.

The Ebola Clock

In early May, a nurse showed up sick at a hospital in Bunia, a city of half a million people in northeastern Congo's Ituri Province. She had a fever. She was bleeding. She deteriorated rapidly and died. The samples collected from her and her contacts were first run through a standard Ebola test in the provincial laboratory. The results came back negative.

The test was looking for the wrong virus.

It took until May 15 for the National Institute of Biomedical Research in Kinshasa — the country's capital, 1,500 miles to the west — to confirm what the provincial lab had missed: the culprit was the Bundibugyo virus, a rare strain of Ebola for which there is no approved vaccine and no approved treatment. Two days later, the World Health Organization declared a Public Health Emergency of International Concern. By then, 246 suspected cases and 80 deaths had already been reported. The outbreak, health officials now believe, had likely been circulating for six to eight weeks before that first laboratory confirmation.

The clock had been running for two months without anyone knowing it was on.

As of June 22, the DRC has confirmed more than 1,000 cases, making this the second-largest Ebola outbreak in recorded history. It has spread from Ituri into North Kivu and South Kivu provinces, crossed into Uganda's capital of Kampala, reached Goma — a rebel-held city of over one million people — and produced imported cases in Germany and France. The WHO's own Director-General described the response as "playing catch-up." By any epidemiological measure, this is already the fastest-spreading Ebola outbreak on record.

The conventional framing of this crisis — a tragedy unfolding in a remote, conflict-ridden corner of Africa — obscures the more uncomfortable story. This is not a natural disaster with a single origin. It is, in meaningful part, a policy disaster with a paper trail.

A Strain That Rewrites the Playbook

To understand why this outbreak moved so fast before it was caught, it helps to understand what makes Bundibugyo different. When the world thinks of Ebola, it is thinking of the Zaire strain — the virus that killed more than 11,300 people in West Africa between 2014 and 2016, and that drove the development of two now-licensed vaccines, Merck's Ervebo and the Johnson & Johnson Zabdeno/Mvabea regimen, as well as effective monoclonal antibody treatments. Those tools have transformed how outbreaks are managed. Ring vaccination, in which a rapidly deployed vaccine is given to all contacts and contacts-of-contacts of confirmed cases, helped contain the 2018–2020 DRC outbreak — the second-deadliest in history — within two years.

None of those tools work against Bundibugyo. The vaccines were developed specifically against the Zaire species. The licensed treatments showed efficacy against Zaire but not against Bundibugyo's different surface proteins and genetic structure. When emergency response teams arrived in Ituri, the medical countermeasures toolkit that the world had spent a decade building was effectively unavailable. Field diagnostic tests designed for Zaire returned false negatives on Bundibugyo samples, which is why the Rwampara Provincial Laboratory initially cleared infected patients. Response, in the absence of approved drugs or vaccines, falls back to the fundamentals: isolation, contact tracing, safe burials, personal protective equipment, and supportive care. Those fundamentals require staff, infrastructure, and sustained investment over time. They require exactly what the United States spent the last year dismantling.

The Surveillance Gap

In a March 2026 memo, a USAID official estimated that funding cuts could produce a worst-case scenario of more than 28,000 Ebola cases over one year. The memo was written before this outbreak began.

Health officials now believe the Bundibugyo virus may have circulated undetected for six to eight weeks in Ituri before laboratory confirmation. The International Rescue Committee, working in the region, described surveillance systems as "catching up with transmission that has likely been occurring for some time." Ituri's health infrastructure was already strained by years of conflict and population displacement. What it also lost in 2025 was a significant layer of American-funded support.

The dismantling of USAID — which the Trump administration dissolved in 2025 and folded into a reduced State Department portfolio — removed a network of partners who, in previous outbreaks, handled logistics that are easy to underestimate: setting up treatment clinics, importing ambulances, transporting laboratory samples from remote health zones to confirmed testing facilities, staffing isolation units, tracing contacts of suspected cases. According to CNN reporting, a senior State Department official denied that any of these changes hampered the outbreak response, stating that "there was no specific person or program associated with USAID in this region that would have detected this." Aid workers on the ground told a different story. "Our funding and our teams in east Africa, central Africa are definitely depleted," one CDC expert said. The United States had also withdrawn from the World Health Organization at the start of 2025, severing formal coordination mechanisms that govern how outbreak data and response support flow between countries.

At NIH's Frederick, Maryland laboratory — a facility purpose-built for exactly this kind of crisis — Ebola research was ongoing until last year, when the lab was shuttered. Staff were laid off. Research into Bundibugyo-specific treatments, which requires access to such high-containment facilities, was halted.

The State Department has since mobilized $270 million for the current response, including $50 million to CEPI to accelerate Bundibugyo vaccine candidates from Moderna, IAVI, and the University of Oxford. It is worth noting that vaccine development timelines measured in months do nothing for the 1,094 people already confirmed infected as of June 22.

The Leadership Vacuum

When the WHO declared its emergency on May 17, the top official responsible for coordinating the American response was HHS Secretary Robert F. Kennedy Jr. He had been on a midterm political tour — visiting Ohio and California to promote his Make America Healthy Again agenda — when an ABC News reporter asked him, on the sidelines of an EPA event, whether he was worried about Ebola. "Yeah, we're working on it," Kennedy said. He did not respond to a follow-up question about what message he had for Americans who were concerned.

A New York Times investigation published in June, based on accounts from current and former HHS staff, described Kennedy as "checked out" — more preoccupied with his campaign against vaccines and his personal health agenda than with managing a department that oversees the CDC, NIH, FDA, and 40 percent of American healthcare through Medicare and Medicaid. Critics cited in the report argued that Kennedy remained detached from HHS operations while Ebola spread in Central Africa. Kennedy called the piece a "hit piece" sourced by "disgruntled" ex-employees and published an 870-word rebuttal on X.

What is not in dispute is the structural condition of the agency he leads. The CDC has lost more than a quarter of its federal employee workforce since January 2025 — more than 1,000 positions eliminated, with hundreds more on administrative leave, and further losses through resignations, retirements, and contract nonrenewals. In an all-staff survey of the CDC's National Center for Emerging and Zoonotic Infectious Diseases — the division directly responsible for Ebola response — 50 percent of employees reported "somewhat low" or "very low" personal morale. Eighty percent of the CDC's top positions were reported vacant. The agency currently has no permanent director, no permanent head of the FDA, and no permanent Surgeon General. The NIH director is serving as acting CDC director under arrangements that have already exceeded the legal tenure limits defined by the Federal Vacancies Reform Act.

During the 2014–2016 West Africa Ebola epidemic, the CDC conducted more than 3,000 deployments of public health professionals to the affected region. In a survey of 604 current and former CDC employees conducted between February and April 2026, 604 out of 605 respondents said the administration's changes had reduced the CDC's capacity to respond to a public health emergency. More than 99 percent. One dissenting respondent.

In the immediate response to the 2026 outbreak, CDC's acting NCEZID director Chris Braden held an all-staff meeting on May 28 encouraging employees to volunteer for frontline deployment and airport screening duty. He told them the response was "going to go on for quite a long time." The agency, he acknowledged, needed to recruit people to screen passengers at airports as an "immediate need" — suggesting that the pipeline for emergency-trained personnel had run thin enough that the response was relying on voluntary mobilization rather than standing capacity.

The Double Infrastructure Cut

One of the subtler threads running through this outbreak is the connection between HIV infrastructure and Ebola preparedness. They share the same networks.

The same health workers who conduct HIV surveillance in the DRC region also perform broader disease monitoring. The same laboratories that process HIV viral load tests also handle specimen transport for other pathogens. The community health workers funded by PEPFAR — the President's Emergency Plan for AIDS Relief — build the trust and local presence that enables contact tracing during outbreaks of any kind. As one CDC expert told CNN during the Ebola response: "The same staff and systems that help stop HIV epidemics are also those that often halt other epidemics. Our funding and our teams in east Africa, central Africa are definitely depleted."

Against that backdrop, the Trump administration announced on June 18 — with this Ebola outbreak already active and accelerating — the initiation of a "phased drawdown" of PEPFAR funding for South Africa. The stated justification was South Africa's "failure to make demonstrable progress on policy requests by the administration." When congressional aides pressed State Department officials about what those policy conditions were, they described demands that Pretoria reduce its partnership with Iran, end Black Economic Empowerment policies, and address the "Kill the Boer" chant — a reference to an anti-apartheid song. "None of these asks have anything to do with health," a Senate aide said. "They're all political."

South Africa has approximately 8 million people living with HIV, the largest such population in the world. PEPFAR had been providing up to 17 percent of the country's HIV funding and paying the salaries of roughly 15,000 health workers. UNAIDS Executive Director Winnie Byanyima called on the U.S. to reconsider, warning that the global aid withdrawal risked reversing decades of progress against HIV. The State Department's position was that PEPFAR "was never intended to be permanent" and that South Africa "is a middle-income country and is more than capable of supporting its own health programs."

The argument is technically accurate and strategically counterproductive simultaneously. The logic that a country can sustain its own health programs ignores what happens in the gap between the withdrawal of external support and the establishment of a replacement system — a gap that, in public health, is not measured in budget cycles but in infections and deaths.

The Risk That Isn't

The CDC's current assessment of the risk to the American public from this outbreak is low. That assessment is almost certainly correct, and it is worth being precise about why. Bundibugyo virus does not spread through air. It transmits through direct contact with bodily fluids of infected and symptomatic people, which is why healthcare workers and family caregivers have accounted for such a disproportionate share of cases. An American traveler passing through Kinshasa airport presents a categorically different exposure profile than a nurse in an Ituri health zone treating hemorrhagic fever patients without adequate PPE. The CDC has rerouted all flights from DRC, Uganda, and South Sudan to four airports with enhanced screening. An American surgeon infected while treating patients in Ituri was evacuated to Germany for treatment. No cases have been confirmed in the United States.

The low immediate risk to American travelers, however, is doing a great deal of rhetorical work in this debate. It is being used to suggest that what happens in Ituri is primarily Africa's problem — a matter of geography and circumstance rather than policy and investment. The evidence does not support that conclusion. The systems that contain outbreaks in Ituri are the same systems that prevent those outbreaks from becoming global events. When the WHO Director-General warns that his teams are "playing catch-up," he is not only describing a humanitarian emergency; he is describing a structural failure that raises the probability that future variants of this or other pathogens will reach places that currently have the luxury of considering their risk "low."

There is also a more direct American interest at stake. CEPI is now racing to develop a Bundibugyo vaccine using American-funded partnerships and mRNA platforms. That work is urgent precisely because the world spent the last decade building defenses against the wrong Ebola variant — not out of negligence, but because that variant was the one that was killing people. The gaps in the medical countermeasures toolkit were always there. Closing them required sustained investment in research infrastructure, surveillance networks, and international coordination. Much of that investment has now been canceled.

The clock in Ituri started running in approximately February. It ran for two months before anyone could confirm what it was counting down to. The systems that might have caught it earlier — the community health workers, the rapid-response laboratory networks, the coordinated international surveillance — were either underfunded, understaffed, or actively dismantled before the nurse in Bunia arrived at the hospital with a fever that tests said wasn't Ebola, until they confirmed it was something worse.

The outbreak is not under control. The response is playing catch-up. The infrastructure built to ensure the world caught the next one faster than the last one is largely gone.

The clock is still running.
